Rogers Wireless Internet stops working on one computer.?

After runnig my wireless Network for a month it has decided to stop working on me. Im not sure what has changed. We have done everything we can from a factory reset on the modem to diagnostics on all the connections. I have forgotten my password/username for my network, and need help.
